The checklist

What a man's blood test should actually include

A test sold as a men's health check can be anything from 6 hormones to a full screen. These are the markers that change decisions for men, and why.

Testosterone, SHBG and free testosterone

Total testosterone alone can mislead: SHBG decides how much is available to your tissues. Low SHBG can hide a problem; high SHBG can make a normal total meaningless.

LH, FSH, oestradiol and prolactin

These explain why testosterone is low: whether the testes or the pituitary is the issue, and whether oestradiol is running high. Without them a low result has no next step.

PSA, free PSA and the ratio

The free-to-total ratio helps tell an enlarged prostate from something that needs a closer look. Worth a baseline from your 40s.

Haematocrit and a full blood count

Essential if you are on or considering TRT, which can thicken the blood. High haematocrit explained.

ApoB and Lp(a)

Heart disease is the biggest killer of UK men. ApoB counts the particles that cause it; Lp(a) is inherited risk a standard cholesterol test never shows.

Fasting insulin and HOMA-IR

Insulin resistance shows here years before HbA1c moves, and it drives both belly fat and low testosterone.

Kidney function with cystatin C

Creatinine rises with muscle mass and creatine supplements, so muscular men can look like they have poor kidneys when they don't. Cystatin C settles it. Creatine and blood tests.

Liver, thyroid and tumour markers

Liver enzymes including GGT, TSH with Free T3 and Free T4, and CEA and CA19-9 alongside PSA.

Being fair

When TrueVitals is not the best choice

You only want your testosterone checked. Selph's Male Hormone test (£65, 6 markers) or Forth's (£89, 12 markers) answers that for a quarter of the price.

You want treatment, not just a test. Numan's 20-marker test (£118) leads straight to prescribing for testosterone, hair loss and weight loss. TrueVitals tests and explains; it does not prescribe.

You are already on TRT through a clinic. Use their monitoring bloods first and fill only the gaps. TRT monitoring.

You want a check of the basics for the lowest price with a venous draw. Medichecks Advanced Well Man, 44 markers for £159 plus the draw.

By age

What to prioritise in your 20s, 40s and 50s

20s and 30s. A baseline: hormones, iron, vitamin D, liver, kidney with cystatin C, lipids including ApoB and Lp(a) once. Especially if you train hard or have used steroids or SARMs. Blood tests for gym-goers.

40s. Add a PSA baseline and fasting insulin. This is the decade where blood pressure, insulin resistance and ApoB start to move quietly. Health check at 40 for men.

50 and over. PSA with the free-to-total ratio every year or two, kidney function, and the cardiovascular markers. Health check at 50.

How we compared

Methodology, and where this page is biased

  1. Who wrote it. James Smillie, founder of TrueVitals. TrueVitals is one of the options compared, so read this as a well-sourced page from an interested party: every competitor figure is from the provider's own website, dated, and their genuine advantages are stated.
  2. What was compared. Each provider's most comprehensive men's or general panel, plus the main testosterone-only tests, as sold on its own website on 2 November 2026. Marker counts are the provider's published figure; where a page states two different counts, both are shown.
  3. Criteria. Clinical coverage for the question the page answers, sample method (venous scores above finger-prick for hormones and insulin), what the report does with the results, who reviews it, turnaround, and all-in price including any blood-draw fee, before promotional discounts.
  4. Corrections. If a figure here is out of date, email support@truevitals.co.uk and it will be corrected and the date above updated. Reviewed monthly.
FAQs

Common questions

For a full picture, the TrueVitals Ultimate: 140 biomarkers from a venous draw, including PSA with the free-to-total ratio, testosterone with SHBG and free testosterone, oestradiol, LH, FSH, prolactin, cortisol, ApoB, Lp(a), fasting insulin, liver, kidney with cystatin C and full thyroid, read together in a clinician-reviewed written report. £399 including the clinic draw. If you only want testosterone checked, a single hormone test from Selph (£65) or Forth (£89) is cheaper.

TrueVitals Ultimate includes both, with free PSA and the PSA ratio, plus the hormones that explain a testosterone result (SHBG, LH, FSH, oestradiol, prolactin). Many men's hormone tests check testosterone without PSA; check the marker list on any test before you buy.

From the providers' own websites on 2 November 2026: Medichecks Advanced Well Man is £159 for 44 markers plus a £35 to £59 venous draw; London Medical Laboratory Well Man Premier Plus £159 plus a £35 to £80 draw; Vitall Male Total Health Check £299 for 27 finger-prick markers; TrueVitals Ultimate £399 for 140 including the clinic draw.

Yes. Testosterone is highest in the morning, so a sample before 11am gives the most useful result, and a low reading should be repeated before anything is concluded. For the Ultimate panel you also fast for 8 to 12 hours beforehand (water is fine) because it includes fasting insulin and glucose.

For a single hormone it can be adequate, but a venous sample is the standard for hormones, insulin and larger panels, and it avoids repeat kits when a capillary sample is too small. Every TrueVitals test is a venous draw. Finger-prick vs venous.

PSA is a useful marker but not a perfect one: it can be raised by an enlarged prostate, infection, recent ejaculation or cycling, and a normal result does not rule everything out. On the NHS, men over 50 can ask their GP for one. Avoid ejaculation and vigorous exercise for 48 hours before the draw, and take any raised result to your GP. PSA blood test explained.

Everything a man's blood test should cover. One draw.
