Executive health check London: what you are actually getting for the money

A London executive health check typically runs £600 to £1,200, takes half a day, and includes a physical examination, a fitness test, a consultation and somewhere between thirty and forty blood markers. Most of the cost is the half day and the doctor's time, not the science. That can be exactly what you want, particularly if nobody has examined you in a decade. But if what you actually want is to know where your cardiovascular and metabolic risk sits, the blood panel inside those packages is the weakest part of them, and it is the part you can buy properly for a third of the price.

The short answer

Buy the full assessment if you want a doctor to examine you, run an ECG and a treadmill test, and talk it through in person. Buy the blood panel on its own if you want depth: ApoB and Lp(a) rather than a basic cholesterol, fasting insulin rather than waiting for HbA1c to move, cystatin C rather than creatinine alone, a full thyroid panel rather than a TSH, and a complete hormone profile. Ultimate covers all of that for £349. Signature at £799 adds blood pressure and physical measurements at a Randox clinic and takes the count to 230, which is deeper than any package on this list.

Five questions worth asking the provider

Corporate health check packages are sold on the experience, and the specification is usually a PDF nobody reads. These are the questions that separate a thorough panel from an expensive morning.

Question 1

Do you measure ApoB, or just cholesterol?

ApoB counts the particles that actually deposit in artery walls and predicts risk better than LDL. Around one in five people with a reassuring cholesterol result has a high ApoB. Most executive packages do not include it.

Question 2

Is Lp(a) included?

Lp(a) is genetic, does not change with diet, affects roughly one in five people, and is tested once for life. It is the single most useful thing a first health check can tell a 40-year-old, and it is almost never in the package.

Question 3

Fasting insulin, or only HbA1c?

Insulin resistance develops years before HbA1c shifts. Testing HbA1c alone means finding the problem at the point where it is much harder to reverse.

Question 4

Thyroid: TSH alone, or the full panel?

A TSH inside a wide reference range with a free T3 on the floor and positive antibodies is the commonest missed cause of fatigue and low mood in high-functioning people. You need free T4, free T3 and antibodies to see it.

What do I actually receive?

A consultation on the day is useful and forgettable. Ask whether you get a written document explaining every marker in context, and whether next year's results will be compared with this year's, because the trend is where the value is.

When the full assessment is the right purchase

If you are over 50 and have not been examined in years, if you have a symptom rather than a question, if there is a cardiac history in the family and you want an ECG and an exercise test, or if you would simply rather sit with a doctor for an hour, book the assessment. Those are real things a blood panel cannot do, and no amount of biomarkers substitutes for a physical examination. What a panel does better is depth, cost, frequency and the write-up, which is why a sensible pattern for most people in their forties and fifties is a full assessment every three to five years and a comprehensive blood panel every year in between. The mistake is paying £1,000 annually for a morning out and thirty-five markers, and calling it a health check.
