Anti-CCP blood test in the UK

An anti-CCP blood test looks for antibodies against cyclic citrullinated peptides, the most specific blood marker for rheumatoid arthritis. A positive result points strongly to RA and can appear years before joints become swollen. Anti-CCP is included in TrueVitals Signature (£799), read alongside rheumatoid factor, ESR, hs-CRP and ANA.

What anti-CCP shows

In rheumatoid arthritis the immune system makes antibodies against proteins that have been chemically altered (citrullinated) in inflamed joints. Anti-CCP detects those antibodies.

It is far more specific than rheumatoid factor: a positive anti-CCP is rarely seen in people who do not have, or will not go on to develop, RA. A positive result is also linked to a more active, more erosive course, which is why early treatment matters.

Anti-CCP vs rheumatoid factor

Rheumatoid factor (in Ultimate)

  • Positive in most people with RA
  • Also positive with age, infections, other autoimmune conditions and in some healthy people
  • Good first screen, less specific

Anti-CCP (in Signature)

  • Positive in roughly two thirds of people with RA
  • Rarely positive without RA
  • Can appear years before symptoms
  • Both positive together makes RA very likely

Who it is worth testing for

  • Joint pain with morning stiffness that lasts more than about 30 minutes, especially in the hands and feet.
  • A positive rheumatoid factor on a previous test, to see whether it is likely to mean RA.
  • A strong family history of rheumatoid arthritis or other autoimmune disease.
  • Smokers with joint symptoms, as smoking is the biggest lifestyle risk factor for anti-CCP positive RA.

Reading the result

A negative result does not rule RA out: about a third of people with RA never make anti-CCP (seronegative RA), and the diagnosis is clinical. A positive result with joint symptoms needs a GP and usually a rheumatology referral. Your report reads anti-CCP with rheumatoid factor, ESR, hs-CRP and ANA to show whether there is active inflammation as well as the antibody.

Be honest with yourself about what this test is for

When to see your GP now: swollen, warm or stiff joints, particularly in the small joints of the hands or feet, should be seen promptly whatever any test shows. Early treatment of RA protects the joints, so do not wait for a private result.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Frequently asked questions

Is anti-CCP included in Ultimate?

No. Ultimate includes rheumatoid factor, ESR and hs-CRP. Anti-CCP is included in Signature.

Can I have rheumatoid arthritis with a negative anti-CCP?

Yes. About a third of people with RA test negative for anti-CCP. The diagnosis is made from symptoms, examination and imaging as well as blood tests.

Can anti-CCP be positive before symptoms?

Yes. It can appear years before joints become swollen. A positive result without symptoms is worth discussing with your GP so any new joint symptoms are acted on quickly.

What is a normal anti-CCP level?

Labs report it against their own cut-off. A result below the cut-off is negative; a strongly positive result is more meaningful than a weakly positive one.

Do I need to fast?

Not for anti-CCP itself, but Signature includes fasting markers, so an overnight fast with water only is advised.
