The most common form of "doctor review" is a sentence or two written against each result that falls outside the laboratory's reference range. It is better than nothing. But it has two gaps that matter more than they sound.
It says nothing about results inside the range. Laboratory ranges are built from the population, including people who are already unwell. A ferritin of 22 is inside the range and a common cause of exhaustion. A TSH of 3.8 is inside the range and, with a low Free T3 and positive antibodies, is a thyroid failing slowly. Neither earns a comment.
It reads each marker alone. Low-normal ferritin, high-normal TSH and a low Free T3 are three unremarkable lines. Together they are the most common missed explanation for fatigue in the UK. A comment per marker cannot see that; a report written across the results can.
