Free testosterone blood test in the UK

A free testosterone blood test measures the small share of testosterone that is not bound to proteins and is free to act on your tissues. Most tests estimate it from total testosterone and SHBG. Measuring it directly is most useful when SHBG is very high or very low, when the estimate is least reliable. You can add measured free testosterone to TrueVitals Ultimate for £79.

Total, free and calculated

Most of your testosterone travels bound to SHBG (tightly) and albumin (loosely). Only around 1 to 3% is completely free. Total testosterone tells you how much there is; free testosterone tells you how much is available.

Ultimate already reports total testosterone, SHBG and the free androgen index, a calculated ratio. The add-on measures free testosterone in the laboratory instead of estimating it.

When a measured value helps

  • High SHBG. Common with age, low body fat, an overactive thyroid, liver conditions and some medicines. Total testosterone can look normal while free testosterone is low.
  • Low SHBG. Common with excess weight, insulin resistance and type 2 diabetes. Total testosterone can look low while free testosterone is adequate.
  • Borderline results with symptoms. Low libido, fatigue or poor recovery with a total testosterone near the lower limit.
  • On TRT. To check the dose is giving you free testosterone in range, not just a good total.
  • Women. The free androgen index is less reliable at the low levels women have, so a measured value can help with PCOS questions.

What affects free testosterone

Lower

  • Ageing (SHBG rises as total falls)
  • Poor sleep, heavy training without recovery, low calorie intake
  • Opioids and some other medicines
  • Pituitary or testicular causes of low testosterone

Higher

  • Testosterone therapy or anabolic steroids
  • Low SHBG with insulin resistance
  • PCOS in women

Reading the result

Testosterone peaks in the morning, so test before about 11am. A single low result should be repeated before any treatment decision, which is also what UK guidance recommends. Your report uses the lab's own range and reads free testosterone with LH, FSH, prolactin, oestradiol and thyroid results to point to the likely cause.

Be honest with yourself about what this test is for

When to see your GP: erectile problems can be an early sign of heart disease or diabetes, and breast swelling, headaches or changes in vision with low testosterone need medical review.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Frequently asked questions

Is free testosterone included in Ultimate?

Ultimate includes total testosterone, SHBG and the free androgen index, which is calculated from them. Measured free testosterone is an add-on for £79.

Is calculated free testosterone good enough?

For most people with SHBG in the normal range, yes. A measured value adds most when SHBG is very high or very low, or when results sit near the cut-off and symptoms do not match.

What time should I test testosterone?

Before about 11am, after an overnight fast with water only. Testosterone is highest in the morning and food can lower it temporarily.

Should I test free testosterone on TRT?

It is useful, particularly if you feel your dose is not working despite a good total level, or if your SHBG is high.

Can women test free testosterone?

Yes. It can help with PCOS, acne and excess hair, read alongside the full female hormone results in Ultimate.

When will my result arrive?

Some specialist add-ons are sent on to a reference laboratory. Your main report arrives within 4 working days, and if an add-on result comes back later it is added to your report automatically and we let you know.
