Homocysteine blood test in the UK

A homocysteine blood test measures an amino acid that builds up when B12, folate or B6 run low, and high levels are linked to heart disease, stroke and cognitive decline. GPs rarely check it. You can add it to TrueVitals Ultimate for £89 (£488 in total with the clinic draw), or get it included in Signature (£799) alongside 44 other specialist markers.

Add-ons are tested on the same blood draw as your panel and reported inside your clinician-reviewed report, read alongside your other 140 results. They are not sold on their own.

Homocysteine keeps rising in the tube after the draw, so the sample has to be spun quickly. Home visits can't guarantee that, so it is clinic only.

What homocysteine is

Homocysteine is made when your body processes methionine, an amino acid from protein. It is normally recycled straight away, and that recycling depends on vitamin B12, folate and vitamin B6. When any of them run short, homocysteine builds up in the blood.

That makes it two things at once: a heart and brain risk marker, and a functional check of whether your B vitamins are actually doing their job, which a standard B12 or folate level cannot always show.

Why it is worth testing

  • Heart and stroke risk. Raised homocysteine is linked to artery damage, heart disease and stroke, independently of cholesterol.
  • Brain health. Higher levels are associated with cognitive decline and dementia risk in large studies.
  • Hidden B12 or folate shortfall. It can rise before B12 or folate fall out of range, especially in vegans, older adults and people on metformin or acid-reducing medicines.
  • It is fixable. High homocysteine usually comes down with the right B vitamins, so it is a marker you can act on and retest.

What raises homocysteine

Common causes

  • Low B12, folate or B6
  • Reduced kidney function
  • Underactive thyroid
  • Smoking and heavy alcohol or coffee intake
  • Some medicines (metformin, acid reducers, methotrexate)
  • MTHFR gene variants, which slow folate processing

What the evidence says about lowering it

  • B vitamins reliably lower homocysteine
  • Trials show a modest reduction in stroke, especially where folate intake is low
  • Trials have not shown fewer heart attacks from lowering it alone
  • So treat it as a signal to fix the cause, not a number to chase

Typical levels

Most UK labs flag results above about 15 µmol/L as raised. Many clinicians consider levels under about 10 µmol/L ideal. Levels rise with age and are usually a little higher in men. Your report uses the lab's own range and reads homocysteine alongside your B12, folate, MCV and kidney results.

Be honest with yourself about what this test is for

When you need a GP, not a panel: sudden weakness, numbness, slurred speech or chest pain need 999. Numbness, tingling or memory changes that have been building up should be seen by your GP, who can check B12 directly.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Frequently asked questions

Is homocysteine included in the Ultimate blood test?

Not as standard. You can add it to Ultimate for £89, or choose Signature, which includes it.

Why can't I have homocysteine with a home visit?

Red blood cells keep releasing homocysteine into the sample after it is taken, so it has to be spun in a centrifuge soon after the draw. We book homocysteine at one of 19 partner clinics that spin samples on site.

Do I need to fast for a homocysteine test?

Fasting is advised. A protein-rich meal can raise homocysteine a little, and Ultimate also includes fasting insulin and glucose, so an overnight fast with water only gives the cleanest result.

What is a good homocysteine level?

Most labs flag above about 15 µmol/L. Many clinicians aim for under about 10 µmol/L. Your report explains where you sit and what is likely driving it.

How do you lower homocysteine?

By fixing the cause, usually low B12, folate or B6. That might mean diet changes or supplements, ideally after checking which vitamin is short. Retest after about three months to confirm it has come down.

Does MTHFR affect homocysteine?

Common MTHFR variants can slow folate processing and push homocysteine up, mainly when folate intake is low. Your homocysteine level tells you whether it matters for you, which is more useful than the gene result alone.
