NT-proBNP blood test in the UK

An NT-proBNP blood test measures a protein released when the heart muscle is stretched or under strain. It is the standard NHS blood test for suspected heart failure, and in people without symptoms higher levels are linked to future heart risk. You can add it to TrueVitals Ultimate for £109, read alongside ApoB, Lp(a), hs-CRP and kidney function.

What NT-proBNP is

When the heart's chambers are stretched by extra pressure or volume, heart muscle cells release a hormone called BNP and an inactive fragment, NT-proBNP. NT-proBNP lasts longer in the blood, so it is the more stable marker to measure.

A normal result makes heart failure very unlikely, which is why NICE uses it to decide who needs an urgent heart scan.

Who it is useful for

  • People with a family history of heart disease who want more than a cholesterol check.
  • People with high blood pressure, diabetes or kidney disease, which put strain on the heart over time.
  • Endurance athletes and older adults monitoring heart health.
  • Anyone wanting a baseline to track over the years.

What raises NT-proBNP

Common reasons

  • Heart failure or a stiff heart muscle
  • Age (levels rise naturally)
  • Atrial fibrillation and other rhythm problems
  • Reduced kidney function
  • Valve disease or high blood pressure in the lungs
  • Very hard recent endurance exercise

Can read lower

  • Carrying more body weight lowers NT-proBNP, so a normal result is read with that in mind
  • Some heart medicines lower it

Typical levels

NICE guidance uses 400 ng/L as the threshold for referral when heart failure is suspected, and above 2,000 ng/L for urgent assessment. In people without symptoms most results are well below 125 ng/L, rising with age. Your report uses the lab's age-related cut-offs.

Be honest with yourself about what this test is for

If you have symptoms, go to the NHS first: new breathlessness, swollen ankles, waking breathless at night or chest pain need your GP (or 999 for chest pain). Your GP can test NT-proBNP free and arrange a heart scan if needed.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

NT-proBNP on its own rarely tells the whole story. In TrueVitals panels it is reported with these related markers:

Frequently asked questions

What does NT-proBNP test for?

Strain on the heart muscle. It is used to rule heart failure in or out and, in people without symptoms, as a marker of future heart risk.

Is NT-proBNP the same as BNP?

They come from the same parent molecule. NT-proBNP is more stable in the blood, so it is the version most UK labs use.

Is NT-proBNP in Signature?

No. Signature measures troponin T and FABP-3, which reflect heart-muscle stress in a different way. NT-proBNP is an add-on to Ultimate.

Do I need to fast?

Not for NT-proBNP, but Ultimate includes fasting insulin and glucose, so an overnight fast is advised. Avoid very hard exercise the day before.

Can NT-proBNP be done at home?

Yes. Ultimate + NT-proBNP with a home visit is £537.
