Omega-3 index blood test in the UK

An omega-3 index test measures the EPA and DHA in your red cell membranes, a reflection of your omega-3 intake over the last few months. An index of 8% or more is linked to the lowest heart risk, and most people in the UK run well below it. You can add the full omega-3/6 profile to TrueVitals Ultimate for £129.

What the omega-3 index is

EPA and DHA are the long-chain omega-3 fats found mainly in oily fish. They are built into the membranes of every cell, including red blood cells, which live about four months. Measuring the percentage of EPA and DHA in red cell membranes gives a stable picture of your intake, unaffected by your last meal.

The profile also reports your omega-6 to omega-3 balance (the AA:EPA ratio) and trans fat levels.

Why it is worth testing

  • Heart health. A higher omega-3 index is linked with lower risk of fatal heart disease and sudden cardiac death.
  • Inflammation. A high AA:EPA ratio reflects a more pro-inflammatory balance of fats.
  • Checking supplements work. Absorption varies widely, so the same dose produces very different levels in different people.
  • Brain and eye health, where DHA is a key structural fat.

Reading the result

Omega-3 index

  • 8% or more: the range linked with lowest risk
  • 4 to 8%: intermediate, where most UK adults sit
  • Under 4%: linked with the highest risk

What moves it

  • Oily fish two or more times a week
  • Fish or algae oil supplements with EPA and DHA
  • Changes take about three to four months to show, so retest after that

How it differs from your other results

Ultimate already measures your cholesterol, triglycerides, ApoB and hs-CRP. The omega-3 profile adds what those cannot show: whether your diet is supplying the fats that support heart and brain health. A low index with high triglycerides is a particularly useful combination to know about.

Be honest with yourself about what this test is for

Be realistic about what it does: the omega-3 index is a nutrition and risk marker, not a diagnosis. If you are on blood thinners, check with your doctor before starting high-dose fish oil.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Omega-3 index on its own rarely tells the whole story. In TrueVitals panels it is reported with these related markers:

Frequently asked questions

What is a good omega-3 index?

8% or more is the range linked with the lowest heart risk. Under 4% is linked with the highest.

How long do omega-3 results take?

The omega-3/6 profile is processed by a specialist lab, so results follow 1 to 3 weeks after your main Ultimate report. We add them to your report when they arrive.

Do I need to fast?

Not for the omega-3 index, as it reflects months of intake. Ultimate includes fasting insulin and glucose, so an overnight fast is still advised.

I take fish oil. Should I stop before testing?

No. Keep taking it as normal. The point is to see whether your current intake is getting you to target.

Is the omega-3 index included in Signature?

Not as standard. It is an add-on to Ultimate.
