Urine ACR test in the UK

A urine ACR (albumin/creatinine ratio) test checks for small amounts of albumin leaking into your urine, the earliest sign of kidney damage, often years before blood tests change. It is recommended every year for people with diabetes or high blood pressure. You can add it to TrueVitals Ultimate for £39, or get it included in Signature.

What ACR shows

Healthy kidneys keep albumin, the main blood protein, in the blood. When the tiny filters in the kidneys are damaged, small amounts leak into the urine. ACR compares albumin with creatinine in the same sample, which corrects for how concentrated your urine is.

Ultimate's blood tests (creatinine, cystatin C and eGFR) show how well your kidneys filter. ACR shows whether they are leaking. Kidney health is classified using both, and either can be abnormal while the other is normal.

Who it is worth testing for

  • Diabetes or prediabetes. NICE recommends ACR every year in diabetes, as it is the first sign of diabetic kidney disease.
  • High blood pressure, treated or not.
  • Heart disease or a strong family history of kidney disease.
  • Insulin resistance, excess weight or high triglycerides. A raised ACR is also linked to higher heart risk.
  • People on GLP-1 or other weight-loss medicines checking their overall picture.

What the results mean

UK categories

  • Under 3 mg/mmol: normal
  • 3 to 30 mg/mmol: moderately raised
  • Over 30 mg/mmol: severely raised

Temporary rises

  • Hard exercise in the previous day or two
  • Urine infection or fever
  • Menstrual bleeding
  • A raised result is usually repeated before a diagnosis is made

How to give the sample

A first-morning urine sample is best, as it is the most consistent. Avoid hard exercise the day before. Ultimate already includes a urine test, so the same sample is used and there is nothing extra to collect.

Be honest with yourself about what this test is for

When to see your GP: a moderately or severely raised ACR, especially with diabetes or high blood pressure, should be shared with your GP, who can repeat it and consider medicines that protect the kidneys. Blood in the urine or swollen ankles also need a GP.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Frequently asked questions

Is urine ACR included in the Ultimate blood test?

Not as standard. Ultimate includes creatinine, cystatin C, eGFR and a urine test. You can add urine ACR for £39. Signature includes ACR.

Is ACR the same as the urine protein test?

No. The standard urine protein test only detects larger amounts. ACR picks up much smaller amounts of albumin, so it finds kidney damage earlier.

Do I need a first-morning sample?

It is best, because it is the most consistent. If that is not possible, any sample can still be tested and your report will take it into account.

Can exercise affect the result?

Yes. Hard exercise in the previous day or two can raise ACR temporarily, so keep the day before your test light.

My eGFR is normal. Do I still need ACR?

It is worth it if you have diabetes, high blood pressure or heart risk factors. Kidneys can leak albumin for years before eGFR falls.
