ApoE blood test in the UK

An apolipoprotein E (ApoE) blood test measures the level of ApoE protein that helps clear fats from your blood. It is not the APOE gene test linked to Alzheimer's risk; that is a separate genetic test. ApoE levels are included in TrueVitals Signature (£799), read alongside ApoB, ApoA-1, Lp(a) and small dense LDL.

What ApoE does

ApoE is a protein on several types of lipid particle, including the remnants left after triglyceride-rich particles have delivered their fat. It acts as a key that lets the liver recognise and clear those particles from the blood.

When ApoE is not doing its job well, cholesterol-rich remnants linger in the blood, which is linked to plaque build-up.

ApoE level vs the APOE gene test

ApoE level (this test)

  • A blood protein measurement
  • Changes with diet, weight and insulin resistance
  • Helps interpret unusual cholesterol and triglyceride patterns
  • Included in Signature

APOE genotype

  • A genetic test for the e2, e3 and e4 variants
  • Fixed for life
  • e4 is linked to higher Alzheimer's and heart risk
  • Not part of this test

Who it is useful for

  • People with high triglycerides or remnant cholesterol, where poor clearance may be part of the story.
  • Anyone with a strong family history of heart disease who wants the deepest lipid picture available.
  • People whose cholesterol looks fine but ApoB or Lp(a) is raised, building a complete particle profile.

Typical levels

ApoE is reported in mg/dL against the lab's reference range. Raised levels often travel with high triglycerides and remnant cholesterol. Your report reads ApoE alongside your full lipid profile rather than on its own.

Be honest with yourself about what this test is for

If you want your Alzheimer's risk gene: this is not that test. Genetic results can be hard to live with, so consider genetic counselling before an APOE genotype test.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

ApoE on its own rarely tells the whole story. In TrueVitals panels it is reported with these related markers:

Frequently asked questions

Is this the APOE4 Alzheimer's test?

No. This measures the amount of ApoE protein in your blood. The APOE4 test is a genetic test for which version of the gene you carry.

Is ApoE included in Ultimate?

No. ApoE is included in Signature. Ultimate covers ApoB, ApoA-1, Lp(a) and remnant cholesterol.

What does a high ApoE level mean?

It often travels with high triglycerides and remnant cholesterol, suggesting fat-carrying particles are not being cleared efficiently. Your report reads it with your other lipids.

Do I need to fast?

Yes, fasting is advised, as triglyceride-related markers are clearer after an overnight fast.
