Fructosamine blood test in the UK

A fructosamine blood test shows your average blood sugar over the last 2 to 3 weeks, compared with 2 to 3 months for HbA1c. It is most useful when HbA1c is unreliable (anaemia, pregnancy, some haemoglobin variants, recent blood loss) or when you want to see the effect of a recent change in diet or medication. Fructosamine is included in TrueVitals Signature (£799).

What fructosamine measures

Sugar in the blood sticks to proteins over time. HbA1c measures sugar stuck to haemoglobin in red cells, which live about three months. Fructosamine measures sugar stuck to blood proteins, mainly albumin, which turn over in a few weeks. So fructosamine reflects a shorter, more recent window.

When it is better than HbA1c

  • Anaemia, recent blood loss or a transfusion, which change red cell lifespan and make HbA1c misleading.
  • Haemoglobin variants such as sickle cell trait, which interfere with some HbA1c methods.
  • Pregnancy, when blood sugar changes quickly.
  • Recent changes, such as starting a GLP-1 medicine, a low-carbohydrate diet or a new diabetes treatment, where you want to see an effect within weeks.
  • HbA1c and glucose that do not agree, as a cross-check.

What can affect it

Falsely low

  • Low albumin (liver disease, protein loss through the kidneys)
  • Overactive thyroid

Falsely high

  • High albumin or protein levels
  • Underactive thyroid

How it fits with your other results

Signature reads fructosamine with HbA1c, fasting glucose, insulin, C-peptide, adiponectin and albumin. When HbA1c and fructosamine agree, the picture is clear. When they disagree, your report explains which is more likely to be right for you.

Be honest with yourself about what this test is for

Not a diagnostic test for diabetes: diabetes is diagnosed in the UK with HbA1c or glucose. Fructosamine is a monitoring and cross-checking tool. If your results suggest diabetes, your GP will confirm it with their own tests.

A private panel is a good way to get a baseline, understand a result in context and track it over time. It does not replace your GP for symptoms, and anything that needs follow-up is flagged in plain words in your report.

Frequently asked questions

Is fructosamine included in Ultimate?

No. Ultimate includes HbA1c, fasting glucose, insulin and C-peptide. Fructosamine is included in Signature.

Is fructosamine better than HbA1c?

Not in general. HbA1c is the standard. Fructosamine is better when HbA1c is unreliable or when you want to see changes over weeks rather than months.

What is a normal fructosamine level?

Labs commonly use a range of about 200 to 285 µmol/L for people without diabetes. Your report uses the lab's own range.

Can I use fructosamine to check a new diet or GLP-1 medicine?

Yes. Because it reflects 2 to 3 weeks, it shows a change sooner than HbA1c, read alongside glucose and insulin.

Do I need to fast?

Fructosamine does not need fasting, but Signature includes fasting glucose and insulin, so an overnight fast with water only is advised.
